UTICA – Prime Care Coordination has officially opened its doors in Utica, marking one of five of these new facilities in New York State. The new facility celebrated its opening Tuesday at 122 Business Park Drive, Suite 3.
This month, the state made a move toward conflict-free care management for those with intellectual and development disabilities by transitioning Medicaid Service Coordination to new entities called Care Coordination Organizations. Prime Care Coordination is one of those COO’s.
At one point, medical care and behavioral health care operated separately from one another, and this move is meant to break down that barrier. Officials say that rather than having different providers involved in a person’s care, this streamlines the process.
“Prime Care Coordination is a new care coordination for organizations to provide health home care management in 41 counties across New York State, funded by the New York State Office of Persons with Developmental Disabilities and the Department of Health," said Tracy Boff, executive director of Prime Care Coordination.
The organization as a whole will care for more than 7,000 people with intellectual and developmental disabilities. There are five regional hubs for Prime Care Coordination, and they’re located in Central New York, the Finger Lakes, the Mohawk Valley, the Southern Tier and Western New York.
